The practice is currently open for new registrations. Please complete our new patient registration form online or call into the practice (either Ardleigh or Dedham) and collect a new patient pack. Please note that completing and presenting the form or paperwork does not guarantee acceptance onto the practice list. To be accepted you must live within the surgery catchment area.

There has been some media coverage recently regarding patients being able to join the practice of their choice regardless of whether their address was within the practice area or not. The Government brought this change in during 2015, but they are voluntary and most practices in Essex, including Ardleigh, have chosen not to adopt them. The reason for this is we believe that to do so would reduce our ability to offer the same quality of service that we offer now to the patients within our boundaries.
