Aims and objectives
What are your aims and objectives in providing the regulated activities and locations shown in part 3 of this statement of purpose
1.To provide a high standard of primary care for our patients within a confidential and clinically safe environment
2. To ensure that patients are seen by the most appropriate healthcare professional as quickly as possible.
3. To focus on the prevention of disease by promoting good health and prophylactic medicine
4. To provide patients with an experience and environment that is comfortable, friendly, professional and caring; showing our patients courtesy and respect at all times  irrespective of ethnic origin, religious belief, personal attributes or their health problem.
5. To treat the patients holistically, understand and meet their needs, involve them in the decisions about their care and encourage their full participation
6.  To involve other professionals in the care of our patients where this is in the patient’s best interest and with their consent
7. To ensure that all members of the practice team have the right skills and training to carry out their duties competently
8. To develop new ways to educate and inform patients in order to encourage patients to be proactive in their health and wellbeing.
9. To ensure the practice complies with all relevant legislation
10. To provide a learning environment where we train a variety of health professionals.  Continual improvement is an important ethos which runs through our organization

Description of the location
(The premises and the area around them, access, adaptations, equipment, facilities, suitability for relevant special needs, staffing & qualifications etc)
 
The main practice is situated in the village of Ardleigh.  It is a purpose built surgery which has been extended on two occasions.  The surgery comprises:
  • 5 consulting rooms
  • one treatment room
  •  two waiting areas
  • Public and staff toilets
  • Reception area
  • Administration area
  • Staff room & kitchenette
  • Dispensary
  • Storeroom
  • Library
 
The practice is accessible to those with limited mobility, including wheelchair users.  A hearing loop is available for those with hearing difficulties.  Interpreters can be arranged for those who require it, including British Sign Language interpreters.
The surgery is fully equipped to carry out its function and has a defibrillator and oxygen on site.
 
In addition to the three partners, the partnership employs a total of19 staff:
  • 2 salaried GPs
  • 1 Advanced Nurse Practitioner, 1 Practice Nurse with prescribing, 1 Assistant Practitioner
  • 1 phlebotomist
  • 3 qualified dispensers
  • 1 full time practice manager, 1 medical secretary and 8 receptionists
 
The practice holds a GMS Contract with NHS England.  In addition to core services, the practice holds a wide range of DES, LES and PES.  Services include:
  • Cervical cytology screening
  • Contraceptive services
  • Child health surveillance
  • Maternity services
  • Certain minor surgery procedures
  • Childhood vaccinations and immunisations
  • Child health and development
  • Ear syringing
  •  Spirometry
  •  Medication review 
  •  Men’s health

  •  Contraceptive advice, including IUD/IUS and implant fitting
